Summary
Older Australians are more likely to be admitted to hospital than younger Australians, and leaving hospital can be a particularly vulnerable time. The national Transition Care Programme (transition care) provides funding for short-term specialised support to help older people to regain their functional independence after a hospital stay. This In-focus report provides a snapshot of 20,500 people using transition care within a 1-year period. It explores who uses transition care, how it is used, and how the program fits into the broader aged care system.
